What kinds of clients do you serve?

We offer geriatric care management for aging seniors and families who want to address immediate and near-term care needs, those suddenly confronted with an illness or life-altering event, and those who want to enhance their overall quality of life. 

More often than not, we work directly with family members to assess the needs of their loved one and coordinate care services. Our clients vary but the vast majority are 55+ and either in crisis, anticipating a transition, or being proactive about maintaining the overall quality of life.

We assess needs, develop personalized care plans, coordinate care services, and serve as your local advocate when making day-to-day decisions. While we will coordinate home care services, True North Geriatric Care Management does not provide in-home care services.

Yes, our minimum level of service or engagement is one month from initial engagement. Our Crisis Care and Care Management services typically take 14 – 21 days to plan and implement but the average engagement is 3-4 months.

Some long term care policies do cover services provided by a geriatric case manager, however, our geriatric care management services have yet to be covered by Medicare or Medicaid. Within the geriatric care management community, there is ongoing deliberation that qualifying services be covered due to the fact that the profession undoubtedly saves Medicare dollars. 

How much time do you devote to my loved one on a weekly or monthly basis?

While the actual time spent with each client may vary each week (according to individual needs), on average, we allocate 12 hours per month to our Care Management clients and 16 hours per month for Crisis Care clients. This time is spread across care planning, coordination, implementation, and face-to-face time with our clients, their families, and the aging care community.

In limited cases, we are available to assist on a consultative (hourly) basis but most families quickly come to understand why we offer fixed-price monthly service agreements. Our contract states that you may cancel services at any time.

We will work with you to help meet your needs. We are usually able to conduct an initial discovery meeting within 48 hours.
